Contacts referral sources and outpatient therapists to notify them of admissions as evidenced by documentation. Interfaces with CSB staff and outpatient therapists and psychiatrists in the community to ensure continuity of care as evidenced by documentation. Initiates and / or collaborates projects to improve systems and processes affecting program operations and patient care. Participates in on-going clinical training and mentorship, and demonstrates enhancement of risk management skills.</description>
